Adult Evaluation and Review Services (AERS)

AERS provides elderly and disabled adults with free comprehensive evaluations, including medical/nursing, environmental and psychosocial assessments. This evaluation is used to develop a care plan, outlining the services needed to help the individual remain at home, or in the least restricted environment possible.

Services

The Adult Evaluation and Review Services, (AERS) program accepts referrals from persons who are concerned about the welfare of an adult who is mentally, functionally or cognitively disabled. Family members, friends, neighbors, church members, or anyone in the community who has observed and or has knowledge of an adult experiencing some difficulty in their current living situation can contact AERS to make a referral.
